It is a five to seven day fever with a very rapid course of deterioration or recovery. It transferred from person to person through the bite of Aedes aegyti and Aedes albopictus mosquito in the Philippines. The key to prevention is through the prevention of mosquito bites of infected mosquitoes and the prevention of mosquito bite is by keeping mosquito away from your homes. Do not let mosquito enter your house through screening of windows and doors and other possible entry points. There should be no standing water inside your house and in the immediate vicinity. Water attracts mosquito for humidity and egg laying. Mosquitoes are also attracted to dark corners of the house like underneath the tables, sinks, behind cabinets and other dark humid areas. Mosquito bites can also be prevented by wearing long sleeved shirts and long pants. If ever mosquitoes get into your house Insecticide spraying is one of the options especially when there are cases of dengue in your area. Remember….mosquitoes can kill humans through introduction of dengue virus into your system.

Bronchial Asthma

Bronchial asthma season is near. Many asthma occur in the cold months of the year. In the Philippines it is from November to January. Asthma can be differentiated from other respiratory diseases with difficulty of breathing through listening to ones breath. Wheezing in asthma is heard during the expiratory phase. Prevention of attack of asthma is by preventing exposure to triggering factors like changes in weather, dust, pollution, scents, and some foods. Different people have different triggering factor. In some people even emotional problems can trigger an attack. There are many forms of treatment for asthma. Medicines for asthma can be in the form of tablets, syrup, inhaled via nebulizer or gas propelled inhalers. Form of treatment can be suited to one's convenience. In many cases asthma will disappaer at a certain age. In other cases it can have daily weekly or monthly attacks. It is important that one identifies his triggering factor in order to prevent the attack. Recurrent attack will lead to emphysema that is over stretching of lung tissue which will lead to more debilitating attack. Exercise is good for asthma during periods when there is no attack. Asthma sufferers has to rotate his medications every week in order to get the full benefit of the medicine since tolerance to the medicine occurs when the same medicine is taken daily for a week.Fruit juice and plenty of fluids has to be taken during the attack. In the elderly asthma has to be differentiated from difficulty of breathing brought about by heart diseases and other systemic illnesses.

Understanding Our Body

Our body has its method of maintaining balance or health. Overworking will make you tired. Our body is signaling we are overworking and we have to take rest or it will suffer from imbalance or disease. Sweating means our body is overheating and since sweating causes loss of water you will feel thirst and we need to take fluids. These are workings of the human body that we need to understand in order to remain healthy. It is important that we understand our body in order to be in balance or healthy. We should study and learn from our body. For instance let’s study hunger. Among the diabetics there is huge amount of food (sugar) in the blood yet there is always hunger. If the diabetic don’t know this he will satisfy the feeling of hunger by eating and this aggravates the condition. The simple feeling of hunger among the non-diabetics means there is little food in his system and food has to be taken. The amount of food needed need not be too much. Overindulgence in food may bring other conditions like diabetes, hypercholesterolemia, and the like. Hunger has to be satisfied by food but just at the right amount enough to replace the energy spent on performing activities that is there is no excess amount that will make you gain weight. There are still other human body manifestations that will alarm us when there is problem in our system. They will be tackled next time.

Arthritits

Cold season is coming and many of the pain of athritis will return. Arthritis is the inflamation of joints. There are many forms of arthritis. It can be osteoarthritis, gouty arthritis, infectious arthritis or rheumatoid arthrtis. Not all joint pain however is from arthritis. There are pain or swelling from the cartilage, tendons, from the muscle near the joint or from the bone itself. Differentiating these will need a physician. The common medicine for arthritis is for the relief of pain and swelling. These medicine have side effects like gut bleeding, allergy, or some renal or heart effects. Medicine intake especially long term intake has to be careful.
Swollen or painful joints need rest but they have to be bent occasionaly in order to prevent frozen joint. External heat can help as well as massage arround the swollen joint. Massage has to be gentle. It will help improve circulation thus removing offending chemicals as well as bringing fresh blood into the area. Internal heat can be also generated by wraping the swollen joint with several layers of towel. It has to be loose to enable good blood circulation into the site. This has helped me and my patients relieve the pain. Another modality of treatment is acupuncture. I have been doing it and it works most especially in removing pain.

Keeping Healthy

Everybody want healthy body and mind. This will give a little relief with our mental activity but be sure you should have something to remind you where you stop temporarily. This is also true with our physical activities. The government, national and local cannot do anything effective to control dengue. We are just waiting for its natural decline. Hopefully the weather cooperates. We are now at experiencing less rain but there are intermittent downpours. These will aggravate the problem since the vector mosquito will survive longer. The temperature at present is very conducive to the longevity of the mosquito.

The most effective way to prevent dengue is eliminate mosquito inside houses and other human habitations like schools and offices. Every household has to be conscious to the presence of mosquitoes. Dengue carrying mosquitoes prefers to stay inside the house/buildings preferably in dark humid corners. They have to go out of their resting places when they are searching for blood meal and water containers in time they are going to lay eggs. Attacking mosquitoes in their resting places inside the house/building needs a simple handy commercial insecticide spray plus removal of water holding containers. It is that simple. Anytime you notice a mosquito in a corner of your house get that spray and aim it at the mosquitoes. For a long lasting easy mosquito control screening of house is the most appropriate. In this way you prevent entry of mosquito as well as other insect like flies thus preventing other diseases as well.

Abdominal Pain

Abdominal pain has many causes. There are several organs in the abdomen that can be the source of pain. The most common is pain from the small intestine brought about by huge amount of gas or flatulence. Gas in the small intestine is produced by the bacterial flora of the intestines. There are foods that causes increase production of gas by the intestinal bacteria. Beans is one of them as well as hotdogs and watermelon. The pain produced by gas in the intestine is colicky, intermittent, severe and is unbearable. There are medicines that will relieve the pain and there are medicine that will left the gas absorbed and turned to liquid state. A simple procedure is to lie on the left side. This will bring the cecum or the junction of the small and large intestine be on the upper side. Since gas normally float it will enter the largeintestine and it will pass easily as flatus thus relieving the pain. Lying n left side for about 30 minutes to one hour will help reduce abdominal pain due to gas. Be careful with your body.

Foreign Body in the Eye

It is very common to have a foreign body lodge into our eyes. It can happen when we ride open air vehicles or when strong winds blows through a dusty field. The usual instinct is to rub the eyes. This is big mistake. If the foreign body is rough, the cornea(surface of the eyes) will get hurt and it will be susceptible to infection. It may even injure the cornea. It will heal as an opaque area in the eye. This the scar of the eyes. If the foreign body consist of iron materials, it will elicit a very bad reaction that will cause severe inflammation and may cause blockade of light into he eyes.

The best thing to do in case foreign body gets into the eyes is to keep the eyes close for several minute maybe 3 t five minutes. This will cause lacrimation and later on a mucinous material will coat the foreign body which will no longer be rough to the cornea. It will then be carried to the inner angle of the eye and become what we call in Pilipino "muta". Nice ha!This is how the eyes protect itself from rough foreign body. Take care of your eyes. In the tropics the heat starts to rise by ten in morning until three in the afternoon and then starts to taper off. The hottest part of the day is unbearable and we need to cool. To keep us cool during summer, avoid the sun at the hottest part of the day by doing our activities indoor or under the roof. If we stay indoor we can keep the ventilation better by opening the doors and windows wide open. This will facilitate air circulation since hot air moves up and in the process air is suck into the room. Bathing is one thing that can cool down the body as long as there is enough water in the tap. We can take a bath as many as we want or we can dip in the pool all day long. More frequent bathing, though, will make our skin dry. Prevent it by not using soap or use soap sparingly. If we bath more than once a day, it is advised to use soap only on areas that sweat a like like the armpit and the inguinal areas since body odor emanates from these areas. Long cold bath before going to bed will give you a very comfortable sleep. Sweat will attract mosquitoes and will bother you in our sleep. Always use mosquito net even during summer. They bite most at the middle of the night.

Activities that cannot be avoided while the sun is high need to be planned well. Use sun protecting lotion and drink plenty of water. The best guide to see if you body needs more water is the thirst and the amount of urine voided. Sometimes we forget to take water when we are busy though we are thirsty. Urine color, amount and frequency will guide us about the water content of our body. If the body lacks water the urine is less frequent, low in volume and highly colored. If we notice this, water or fruit juice has to be taken to replenish our body water. It is best to replace body water with water plus fruit juice. Plain water in large amount will create imbalance on our electrolyte. This will be observed as feeling of fatigue. Fruit juice will give us potassium and little sodium electrolytes. Salt or sodium chloride is lost in the sweat and it should be replaced if we sweat a lot through eating salty food.

Get outside once the sun has set and feel the cool breeze of the afternoon. Socialize or engage in some sports activities. Summer sports fest is only around the corner. Joining the team or be a mere spectator is a cool activity at this time of the year.

ANTIBIOTICS

Bacterial infections are treated by antibiotics. Some examples of bacterial infections are boils, furuncles, gingivitis, tonsillitis, some diarrhea, urinary tract infection, tuberculosis and many more. There are many kinds of bacteria and there are antibiotics suited for each kind of bacteria. There are antibiotics that are effective in a certain group of bacteria. Bacteria can develop resistance to antibiotics. Resistance of bacteria to antibiotics usually develop if the bacteria is exposed to that specific antibiotic at a level that is not enough to kill the bacteria. It will seen as ineffectiveness of treatment or persistence of infection or symptoms. The danger in here will be transfer of this bacteria to other person and into the community. This is now happening in tuberculosis and other infections.
Misuse of antibiotics through self medication, improper dosing, and over use is happening. Abuse of antibiotics through self medication is largely the cause of many antibiotic resistance. The consequence of antibiotic resistance will be the cost of treatment. Even if a bacteria is resistant to a particular antibiotic there are still other antibiotics that will be used. The newer antibiotic will be appropriate but the price is exhorbitant. It is fine for patients with the ability to buy however the people who are more affected with infections are the poor. Decision to the use of antibiotics is therefore must be based on the expert in the use of antibiotics and they are the doctors. Antibiotics must not be use for self medication. Expert advise of the physician must be sought before using antibiotics.
The human body is capable to overcome the infection given appropriate response time. In many infections more than a week is needed to develop antibody against a specific infection. The body must be allowed some time and enough period of exposure to the bacteria in order to mount an effective immune response. Delaying use of antibiotics for 3 to 4 days provide enough exposure for the immune system to craft a specific antibody. In life threathening situation however antibiotics use must not be delayed but this situation is best handled by a doctor. Self medication with antibiotics must never be practiced.

Healty Eating Habits

"YOU ARE WHAT YOU EAT" a common quotation we see around. We really are. Many of the diseases come form the food we take either in volume or quality. There are four uses of food, namely for growth, repair, maintenance and energy. At this time we need repair materials. Growth and repair are provided by protein foods like meat, fish, poultry and legumes. Maintenance of the body is provide by the fluids, mineral and vitamins that comes with foods together withe the growth, repair and energy foods. Energy is needed by the body to generate heat and move the muscles. There are two general sourced of energy, the carbohydrates and the fats. If they are lacking then the body shifts its source by degrading protein to extract energy, but this will lead to production of acids and other more metabolites excreted via the kidneys, and it is not good for the body. Here comes the tricky part.
The waste products of the body and its excretion. The waste product of carbohydrates are carbon dioxide and water. They are excreted via the lungs when we breath. Carbohydrates are exemplified by rice, wheat, and root crops. Oils and fats are the other sources of energy. Oils and fats are also made up of carbohydrates, hydrogen and oxygen similar to carbohydrates but they are in longer chain. Is has more complicated route of metabolism and on some stages acids or ketones are produced. They are also harmful to the body. Cholesterol, triglicerides and lipids are fats that cause the clogging of arteries that produces heart attack and stroke. Now we can say that the healthiest to eat is carbohydrates but it should be accompanied by a small amount if protein and fats. The total volume has to be adjusted to the kind of activities we do. Manual workers can have more and office worker who sits all day have to eat less. Meals has to contain vegetables and fruits for the minerals and vitamins to complete the need of the body. The body is also stressed by digesting the extra food we overeat as well as excreting the extra waste product via the lungs, the kidney or the intestine. Eat less and live longer.

Health and Massage

There is proliferation of massage parlors and spa nowadays. This is a phenomenon brought about by stress in today's life. Stress, both mental and physical, brings about changes in our capacity to perform. It can make us less productive, or can make us feel uncomfortable ans sometimes manifests as symptoms of disease like hypertension, insomnia, body aches, headaches and several other manifestations. One common effect of stress is tense muscles. It will manifest as ache to a particular body part but the common denominator is contracted muscle. Muscles are every where in the body and the ache it causes is referable to the particular are where is is located. It can be a large muscle like the thigh muscle or it can be a small muscle like the muscles inside the eye.

Pain in contracted muscle is brought about fatigue of muscle cells which will be followed by retention of carbon dioxide. The retention is brought about by inadequate blood circulation of that particular muscle. If a small group of muscle is affected we may not notice that the ache is emanating from the muscle. A good example is eye strain. It is due to overworked muscle inside the eye but will manifest as frontal headache sometimes associated with dizziness. Another good example is headache from fatigued muscles of the head.
Massage is pressing the muscles basically. This will press the muscle and squeeze the blood literally in the pressed area. Once the pressure is released there will be flow of fresh blood into the area since the body circulation is a one way affair. Carbon dioxide will then be carried out by the squeezed blood into lungs which will be exchanged for oxygen. The amount of pressure during massage varies. It can be light as a soft breeze or can be tight as a pound of a hammer. The best massage for tight muscles is a gentle massage. Enough pressure to squeeze the blood out of the muscle is needed to relax the tight muscle. Too much pressure will damage the muscle and too light will cause further muscle contraction hence additional carbon dioxide to the burden. Pressure of massage will also vary with the location of the contracted muscles as well as size of the muscle mass. Muscles located deeper will need more pressure and muscle located nearer the surface or has smaller size will need gentler pressure. The best massage will make you feel relaxed and relieved with the aches gone. Have a nice massage. Keep stress out. Live a relaxed life. Happy Living!

Exercise and your Body

Physical exercise is necessary for good circulation of blood. The form of exercise must be suited to your need and your body has to be ready to the form of exercise you need or you want. Exercise can likewise be classified into isotonic and isometric exercise. The former is exemplified by jogging and the later by weight training. Starting and exercise program should be planned depending on your purpose. The general rule is to start lightly and and increase the intensity gradually over a period of time. For every session there should be a warming period like stretching and slow jogging. At the end of exercise session there should also be a cooling period like stretching and slow paced walking over a period of 15 to 20 minutes. Cooling period prevents sudden shut down of circulation in your muscle and removes much of the carbon dioxide accumulated. This prevents muscle aches and fatigue. Keep on walking slowly until the perspiration stops. This indicates that the carbon dioxide has been removed and your body temperature has cooled down. Bathing immediately after exercise is a taboo among Filipino oldies. It has some basis. Bathing will cool down your body immediately and lessens circulation of blood to your muscles. In the laboratory, heating up the specimen is use to uncoil the DNA molecules, maybe sudden changes in body temperature have the same effect. (We need not ignore the wisdom of the old folks.)
Exercise has an effect on cardiac function. It improves collateral blood vessel. In will also increase muscle mass of the heart. Weight training will increase muscle mass but this will increase resistance to blood flow causing the heart to work a bit harder. This is the reason why people with hypertension are not advised to do weight training. They can only do jogging or distance running or bicycling and they have to start gradually.
Exercise to enhance looks has be be planned. Assessment of the outside appearance of the body is needed before the work out. The general rule is to use lighter weight initially and adding more after several sessions. The area that need to be enhanced has to be identified and the movement in that identified area will be the basis of the workout. To determine this area, identify the muscle movement responsible and add weight to that movement. Enhancing the body form should also include the smaller muscle underneath.
Exercising to increase the power of the muscle needs another form of training. Power lifting will involve increasing the power of the muscle in the back, lower and upper extremities, neck and hands. It also needs coordination. It is a separate discipline. Coaching is necessary for this training.

Skin Care

Skin or integumen covers our body. They protect us from water loss and entry of microorganisms into our body. Taking care of our skin is very important. Skin can make us look older or younger. The best thing for the skin is hygiene. Bathing reduces the population of bacteria in our skin and prevents infection like abscess or boils and keep us smelling good. Soap during bathing has to be left for about five minutes in order to kill the bacteria. Soap is germicidal. There is no need for germicide containing soap. Soap can make our skin dry especially those in the sixties. If the skin is dry due to bathing we can use soap with moisturizer.

Rest is also important for our skin. Six to eight hour rest is needed. Skin in the face will make us look older if the skin is dry. To make us look younger we can massage our face with round movement in the cheek and forehead. Massage it for about ten minutes using mild soap or soap with moisturizers. Do it at night before we rest. Avoid touching our face with unwashed hands.

Skin in the body and extremities also needs care. Do not scratch your skin when itchy and keep your skin dry. Change clothes as often and do not wear unwashed clothes. Avoid applying scents and other chemicals that may cause irritation and allergy. Do not wear tight clothing because the skin will react by producing more pigment. Use light loose clothing while sleeping. Eat plenty of fruits.

Care of digestive system

Digestive system starts from the mouth to the anus.Problem in each segment gives different symptoms. Problem with the esophagus may present as difficulty in swallowing or sometimes a little choking sensation. It is called esophagitis. It is caused by reflux of the acidic content of the stomach. Reflux can be due to coffee, too much food intake, intake of very hot soup, or even lying down when the stomach is full. Further down the esopahgus is the stomach. This secretes acids to help digest proteins in our food. Lesion in the stomach is called gastritis and when it is deeper it is called ulcer. This lesion gives hunger feeling although you have just taken food. To take care of your stomach, the simplest way is not to sleep when your stomach is full. The stomach also needs rest during our sleep. Don't let your stomach work while you are sleeping. Try to sleep with your stomach is full. The following morning you will get a feeling of hunger. Do it every night and it will become more severe. Give your stomach a rest while you sleep. Do not sleep while you are full. Wait for about an hour or two after dinner before going to bed. Ulcer can also occur in the deudenun. Intestines just need clean food in order to be healthy. Diarrheas are manifestation of problems in the intestines. Most of them are due to infectious organism from the food that we take. Other disease like cancer in any level of the digestive system will also manifest differently but usually there is a mass which will produce obstruction or loss of weight or changes in bowel movement. Eat healthy. Eat foods with high fiber content and just enough for your daily energy and body repair need. Increase intake of fruits. Excess food will give you problems like diabetes, hypercholesteronemia, hyperlipedemia and many more. Eat just enough. Share you food to the less fortunate like me (just kidding). Happy living!

Disease Transmission

Diseases can be classified into communicable and non-communicable diseases. Some examples of non-communicable diseases are hypertension, diabetes mellitus, and sroke. Some examples of communicable diaseases are cholera, tuberculosis, hepatitis, diptheria, anthrax an dmany more. Disease agents has to get into the body inorder to cause disease. The portals of entry are damaged skin, through the mouth, airway, urethra, skin pores, mucosa of the eyes and nose. Some can penetrate intact skin like schistosomiasis while other are through the bites of insect vectors like malaria and dengue. To keep ourself healthy we need to guard these portals of entry. Clean skin, clean air, food and water. It is a must that we live in a clean environment. No dust, smoke, damp, or insect breeding places. We are the only person who can clean our own surrounding or we have to pay other people to keep our surrounding clean. We inahale 16 to 20 times per minute and each inhalation carries a risk of infection from airborne microorganism. This is the reason why there are more respiratory tract diseases than diarrheas or urinary tract infections. We only take water or liquids 8 to 12 times a day and food 3 to 5 times a day. The risk increasesa as we increase our expossure to the disease carrying materials like air, food or water. keeping these three (air, food and water) is good enough to keep as ree from diseases carried by them. it is therefore necessary that we have to keep pur surrounding clean at all times. This message is very much applicable in the Philippine setting where there is overpopulation and the climate is conducive to proliferation od bacteria.

Dengue Fever

Dengue fever is one of the diseases transmitted by mosquitoes.
It is a disease with a very short course but with very drastic course sometimes.
There are four serotypes of dengue. Infection with one does not confer immunity
to the other types. Infection with one type however gives lifetime immunity to
the specific serotype. The disease starts with a fever after about a week
from the bite of infected mosquito. The fever is accompanied by constitutional signs
and symptoms not specific for dengue infection. Signs and symptoms includes fever, abdominal pain, vomiting, retro-orbital pain, malaise. The home management of dengue is high intake of fluids like fruit juice or soft drinks. Oral rehydration fluids for diarrhea can still be used. Dengue and diarrhea are similar in the sense that the effective circulating body fluid is reduced though in dengue the fluid is still inside the body though in the extra vascular area. After a few days a patient can develop bleeding in the form of petechiae, or gum bleeding or epistaxis. Sometimes the bleeding is in the internal organs like the gastro-intetinal tract.
Signs and symptoms considered danger signs are persistent vomiting, drowsiness, lethurgy, cyanosis, seizure, shock, absence or weak pulse, prolonged bleeding, severe abdominal pain and loss of consciousness. Patient has to be taken to the hospital if any of the danger signs is observed.

Preventing dengue is through prevention of mosquito bite particularly Aedes aegypti and Aedes albopictus. The dwelling area has to be free from stagnant clean water to prevent breeding of mosquito. Mosquito needs water every 3 to 4 days to lays eggs. If there is no available stagnant water in your dwelling area the mosquito will look for one maybe in the immediate vicinity or in the next house. Insecticide
maybe useful but removal of stagnant water is the best. You will even notice that no mosquito bothers you inside your home.

Antibiotic abuse

I have seen unusual number bronchopneumonia just this past 2 weeks. I have been using this antibiotic for about ten years already. This is a result of antibiotic misuse. Patient already self medicate with this antibiotic. The health personnel also learn to use this antibiotic but the problem is the dosing. Most of the time the dose is not appropriate. I have to stop using Co-Trimoxazole now, instead, more effective and expensive medicine will be needed. I hope the patients can afford this medications. The present situation is stressfull. Stress can come from the workplace or at home. Stress will make your blood presure rise, gives you sleepless nights or makes you loose focus. Stress is a mental state. It is just a temporary state and you can manage it. When you are under stress youd mind is active i.e. thinking. managing your minds is through diversion of your thinking to such activities like physical activities like sports, hiking, eating and other usual activities you do when you are under stress. Managing you active mind need diversion of your thoughts as well. To do this you need to follow these steps: (1) Close your eyes (2)Breath slowly (3) Listen to yur breath (4) In you thought say rising during inspiration and say falling during expiration, If something enters your thought which usually is, remove it from your thought and go back to step number 4. After some time you may fall into sleep. Even if you do not fall into slumber you will find your strss is gone and you get more energy to move on. Practice this every time you need it. It is the same technic if you cannot go to sleep at night. This can be done while sitting or lying down.

Diabetes

Diabetes is the condition wherein the blood sugar is high. It could be due to lack of insulin or over replacement of blood sugar from body reserves. High sugar content of the blood increases its viscosity hence circulation in the small blood vessels, particularly capillaries, are affected. Poor circulation means poor supply of material to cells like sugar and oxygen. These cause cell hunger and the cell shifts to other source of energy within the cell, the anaerobic energy source. It is the one that neither use oxygen or sugar but fats or protein. Waste products of this metabolism are acids which are secreted through the kidneys. This overburdens the kidney in the long run. The other affected organs, the nervous system particularly the peripheral nervous sytem, is also affected through the same mechanism...slow blood flow. The symptoms will be numbness and tingling sensation of extremities.
Reduction of blood sugar is the key to longer onset of complications and diet is the most important. To make it easy to comprehend, suppose your blood sugar is 300 mg%, halving your meals will help. Three hundred mg% blood sugar is twice the normal hence halving the intake will help. Exercise will also help as it release adrenalin that will help reduce he blood sugar. It is also dangerous to lower your blood sugar too much. Its sympstoms will be dizziness, weaknes or fatigue assocaited with cold sweat or hunger. At this time you need an emergengy intake of food with high sugar content like softdrinks or sweatened fruit juice. Consult your physician as soon as possible if you have this symptoms. Diabetics must have a regular appointment with their physicians. Once the complication set in... it will not be reversed.
